Corneal Cross-Linking (Keratoconus)

Normally your cornea, the clear outer lens or “windshield” of the eye, has a dome shape, like a ball. Sometimes the structure isn’t strong enough to hold its round shape and it bulges outward, like a cone. This is called keratoconus.

Corneal collagen crosslinking (CXL) is a minimally invasive procedure that can help prevent the progression of corneal ectasias, such as keratoconus and post-LASIK ectasia. The procedure involves strengthening the cornea by promoting the formation of new collagen fibers. This helps to stabilize the cornea and slow down or stop the progression of ectasia.

Indications for Corneal Cross-Linking (Keratoconus)

Keratoconus patients need to be checked every 6 to 12 months by an ophthalmologist. If the condition is stable, treatment with corrective glasses or contact lenses is usually all that's needed. However, if there's evidence of progression, an early cross-linking treatment may be recommended to stop the disease from worsening and potentially sparing the patient from needing a corneal transplant (penetrating keratoplasty) down the road. The treatment is relatively easy and low in cost.

Corneal Cross-Linking Laser

This particular procedure is performed while the patient is under topical anesthesia. Abrading the epithelium (corneal scraping) is the first step, followed by applying Riboflavin (vitamin B) drops to the cornea for 30 minutes. The cornea is then exposed to 365nm for an additional 30 minutes.

The following procedure is to be performed with topical anaesthesia. After an epithelium abrasion (corneal scraping), Riboflavin (vitamin B) drops are applied to the corneal surface. During the follow up of eyes treated with collagen cross linking, very few patients showed any more progression. In approximately 80% of the patients, a regression of the maximal K-values (regression of the keratoconus) was seen.
