Indication for Scleral Fixation

The main indications for this type of treatment include when the lens is not in the right place (subluxation) or has fallen out of place (dislocation), and when there is no lens in the eye at all (aphakia) due to any cause. This can happen after surgery to remove a cataract from the eye, or as a complication of cataract surgery such as when the back part of the capsule that holds the lens in place is torn (bag dialysis), when the zonules that hold the lens in place are damaged (zonular dialysis), or when there is a large tear in the posterior capsular.

Scleral Fixation of IOL

Scleral fixation of the intraocular lens is an alternative to in-the-bag IOL implantation and is performed when the latter is not possible. The two main approaches utilized are sutured and sutureless SFIOL. In the technique of SFIOL, the optic of the IOL is placed in the posterior chamber while the haptic is fixed to the sclera, approximately 1.5 to 2 mm from the limbus - which is the external landmark for the ciliary sulcus. Placing IOLs in the sulcus region is considered safer for both the corneal endothelium and iris.
